Horizon Blue Cross Blue Shield of New Jersey (Horizon BCBSNJ), located in Newark, has announced it will be offering a Dual Special Needs Plan (DSNP) for the upcoming 2017 plan year. The new DSNP plan will be added to the large selection of new low premium and no referral Medicare Advantage plans that Horizon BCBSNJ is now offering during the Medicare’s Annual Election Period (AEP) that opened on October 15.
Horizon BCBSNJ will be offering the Horizon NJ TotalCare plan (HMO SNP) to eligible beneficiaries in Atlantic, Cumberland, Essex, Gloucester, Hudson, Hunterdon, Mercer, Monmouth, Morris, Passaic, Salem, Somerset, Sussex, Union and Warren counties. This plan coordinates benefits for people eligible for both Medicaid and Medicare and features $0 premiums and $0 copays.
“People eligible for both Medicaid and Medicare often have particular health care needs,” explained Erhardt Preitauer, Senior Vice President, Government Programs, Horizon BCBSNJ. “The Horizon TotalCare plan (HMO SNP) provides a single card that helps coordinate care with easy to use benefits that help our members achieve their best health.”
Horizon BCBSNJ’s Medicare Advantage plans, like many of its plans in other markets, emphasize care coordination as one of the most effective ways to improve care quality and reduce health care costs. While receiving the benefits of patient-centered care, consumers purchasing the Medicare Advantage plan will have access to suite of clinical and wellness programs, complementing the plan’s focus on addressing members’ complex health needs and helping them manage chronic conditions and acute-care episodes.
