Children’s Specialized Hospital and Blueprint Honored with 2015 Microsoft Health Innovation Award

BluePrint Healthcare IT and Children’s Specialized Hospital with many locations throughout New Jersey, announce they have been named the recipient of Microsoft Corp.’s 2015 Health Innovation Awards – Patient Engagement category.

The awards, which were announced at the 2015 HIMSS Annual Conference and Exhibition in Chicago, Ill., recognize health organizations and their technology solution partners for using Microsoft devices and services in innovative ways that help enhance the quality of patient care, improve clinician productivity and electronic medical record (EMR) optimization, and streamline clinical and business processes. The 2015 winners are revolutionizing the health industry by creating breakthrough solutions that are empowering healthcare organizations to make a real impact on healthcare.

BluePrint’s Care Navigator™ is a Microsoft Dynamics CRM-based and mobile-enabled healthcare technology platform that empowers real-time, team-based care coordination and communication. With thousands of days under navigation, it extends collaboration across the entire care team to include internal staff, physicians, as well as patients and family caregivers.  Children’s Specialized Hospital deployed Care Navigator™ in their largest primary care practice as part of the medical home model, with the ultimate goal of improving efficiency and effectiveness of communication between Children’s Specialized Hospital and family caregivers.

Children’s Specialized Hospital has had tremendous success improving the coordination of care within their Pediatric Primary Care group.  The Care Navigator™ platform was rolled out in a pilot program which garnered positive feedback from their patient parents and clinicians.

“The Care Navigator™ program allows me to easily coordinate and manage family-centered needs for our patient families,” stated Gina DiGiacomo, Patient Care Coordinator at Children’s Specialized Hospital.  “It also allows families to communicate quickly with his/her Care Coordinator if they have any questions or concerns.”

The parents of these children with special healthcare needs have found the platform to greatly improve their communication.

Nominations were submitted by hospitals, health systems, clinics, public and private health organizations across the world who are applying Microsoft technology to create transformative and highly-effective innovations. An esteemed panel of industry experts selected this year’s winners based on how their innovation represents a forward-thinking development or implementation of a solution that is delivering groundbreaking results and producing better health outcomes for more people.
