CareCentrix and Horizon Healthcare Services, Inc. to Provide Home Care Services for Millions of New Jersey Members

CareCentrix, involved with managing patient care to the home, announces that it has entered into a 5-year agreement with Horizon Healthcare Services, Inc., New Jersey’s oldest and largest health insurance company, to provide Homecare Benefit Management services. The agreement demonstrates the future of home-based transitional care and the central role CareCentrix plays in helping to control the unsustainable rise in healthcare costs. CareCentrix covers 23 million patients nationwide and has a national network of almost 8,000 credentialed homecare provider locations.

Under the terms of the agreement, CareCentrix will provide for the management of Horizon’s Care@Home program, delivering home health services, including durable medical equipment, home infusion management services and traditional home health to its more than 3.7 million members.

The starting date of the new Horizon Care@Home program as described is subject to the New Jersey Department of Banking and Insurance’s review of the management services agreement between the parties.
